The Victim Impact Statement is a form you may complete that describes the financial, physical, and emotional impact the crime has had on you. This form will assist the prosecutor, judge, and correctional agencies with the prosecution of your case. Please complete the VICTIM IMPACT STATEMENT form sent to you by the SPU and return by email to:

You may also print and mail the VICTIM IMPACT STATEMENT (VIS) to:

Finally, you may choose to create an on-line Victim Impact Statement, please go to the Texas Department of Criminal Justice (TDCJ) website and follow the instructions.
